FORM 352. Request for Use of State Aircraft by Cabinet Personnel

GENERAL INSTRUCTIONS. This form should only be used for requests to use state aircraft for out-of-state travel. Please complete the following information and click “Finish” at the conclusion of this form. All efforts will be made to respond within 24 hours of receipt of request. Information about hourly state aircraft rates may be obtained from the KYTC Aviation website or by calling Kentucky State Police. This form is subject to disclosure under the Kentucky Open Records Act, Ky. Rev. Stat. § 61.870 et seq. Any personal identifying information, or other information subject to an exception enumerated in the Act will not be disclosed.
