If you need to obtain a license verification or clearance from KBMIRT for another state license, please forward the form that is required from the other state, along with the required $25 fee (payable by check or money order written to the Kentucky State Treasurer) to the KBMIRT office at 125 Holmes Street, Suite 320, Frankfort KY 40601. The portion of the form that is required to be completed by KBMIRT will be completed and the form will be sent directly to the other licensing agency within 2-3 business days. If no form exists, you may submit a written request for a license verification letter, along with the required $25 fee to KBMIRT; please assure that your request includes what information is required by the other state along with the licensing agency’s address. A formal license verification letter will be sent directly to the other licensing agency within 2-3 business days.
