This form can be used by CPAs, exam candidates, or candidates who have passed the exam and are waiting to satisfy the experience requirement to inform the Board of an address change.
The Board maintains only one address in its files. This address may be either your home or your business. However, as a state agency, the Board is required, if requested, to release the address you provide. Please keep this requirement in mind when you submit an address.
Please complete all information pertinent to the change you are making, as this information will replace what is currently in the Board's files for you.
